Following a successful contract award, we at Calea understand that a robust changeover programme would be agreed with healthcare professionals to ensure a seamless implementation of the new contract. The objective is to provide a smooth transfer or progression onto our homecare service for patients with minimal disruption to all involved.

We have extensive experience with change management and work closely to develop an implementation plan to ensure minimal disruption to all involved. We recognise and understand how change affects patients and carers, we aim to minimise this by working with relevant Trust personnel to formulate an implementation group made up of key representatives from the Trust and our internal teams.

The success of the programme is dependent on:

• Working together as a team
• Accurately and timely transfer of information
• Individual patient needs identified and communicated
• Timescales being identified and agreed
• Close monitoring of the programme
• Clear lines of communication, with regular and planned updates on progress

At Calea we are aware of the anxieties that can be experienced by patients and families in relation to change. It is with this in mind that we have developed a patient focussed changeover plan that is completed in a time sensitive manner with the emphasis being on the patient.
